ID Requirements for All Piercing Appointments

New Hampshire state law requires the following documentation must be presented at the time of all piercing appointments

Requirements for Adult (18+) Piercings

All persons over the age of 18 receiving a piercing must have a valid photo ID at the time of the piercing appointment. Acceptable forms of ID are a valid Drivers License, non-drivers ID, or passport.

Requirements for children/minor piercings (-18)

• A parent or legal Guardian to be present with the minor at the time of the piercing appointment.
• Both the parent or legal guardian and the minor have a valid form of photo identification. A legal state-issued ID such as a driver’s license or passport is preferred. However, if the minor does not have a state-issued ID, a school ID or yearbook photo will be accepted but MUST be accompanied by a birth certificate or legal document/form that shows a birth date (i.e. sports physical, or school portal). 
• The last name of the parent or guardian MUST be the same as the minor. If not a legal document as to why they are different must also be presented.

Examples: marriage license, divorce papers, minor listed as a dependent on a tax report, or guardian’s name matches on their I.D. as listed on the birth certificate.

Siren Body Piercing will prosecute to the fullest extent of the law anyone attempting to use a fake ID or trying to impersonate another person as this is a federal offense.
